Tsitsipas overcame Struff after three intense sets and 1 hour and 57 minutes of play in Madrid.
After the first set favored the German, the Greek made a comeback, significantly reducing his number of unforced errors (4 compared to 17) and showing opportunism in the second set (2/3 break points).
In the final set, the 26-year-old broke his opponent's serve at 3-3 and eventually won the match. He won nearly 87% of points on his first serve. This marks his seventh victory over Struff, with their last encounter dating back to Cincinnati 2024 (victory for the Greek).
Having been forced to retire in Barcelona, the world number 18 is eager to perform well, especially as he also suffered a surprising first-round elimination last year against Monteiro.
He will face Musetti in the next round, following their match in Monte-Carlo (victory for the Italian).
